GOOD FAITH ESTIMATE NOTICE

Your Right to a Good Faith Estimate

Under the No Surprises Act, you have the right to receive a “Good Faith Estimate” explaining the expected cost of your medical and mental health care.

If you are not using insurance, or choose not to use your insurance benefits, you are entitled to receive an estimate of the total expected cost of services.

    •    You may request an estimate at any time

    •    This estimate outlines expected costs based on your treatment needs

    •    Individual Therapy (50 min): $135–$150

    •    Couples Therapy (50 min): $150- $175   

 •    Extended Sessions (90 min): $195

The total cost of therapy will depend on:

    •    Length of treatment

    •    Frequency of sessions

    •    Clinical needs and goals

Dispute Rights

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.

You can contact the U.S. Department of Health and Human Services for more information about your rights.
